1. Introduction
Welcome to UDEA Physical Therapy Inc. By scheduling an appointment or using any of our services, you agree to be bound by the terms and conditions outlined in this document. Please read these terms carefully before accessing or using our services.
2. Services Provided
UDEA Physical Therapy Inc provides outpatient orthopedic physical therapy services, including but not limited to manual therapy, therapeutic exercises, Pilates for rehabilitation, visceral manipulation, neural manipulation, and other related services. All treatments are tailored to the individual needs of each client and are conducted by licensed physical therapists.
3. Appointments and Cancellations
Scheduling: Appointments can be scheduled by phone or email.
Cancellations: We require a minimum of 24 hours' notice for cancellations. Failure to provide sufficient notice will result in a cancellation fee, as outlined in our fee schedule.
No-Shows: If you fail to attend a scheduled appointment without prior notice, you may be charged the full session fee.
Late Arrivals: If you arrive late, your session may be shortened to accommodate the next scheduled client. The full session fee will still apply.
4. Payment Terms
Payment Due: Payment is due at the time of service. We accept cash, credit/debit cards.
Insurance: UDEA Physical Therapy Inc. operates as an out-of-network provider for all insurance carriers, except Medicare. Clients are responsible for understanding their insurance benefits and coverage. We provide detailed super bills upon request, which clients can submit to their insurance companies for potential reimbursement. Any costs not covered by insurance are the sole responsibility of the client.
Refunds: No refunds will be provided for completed services.
5. Client Responsibilities
Medical Information: Clients must provide accurate and complete medical history information. This includes any updates to your health status or changes in medications.
Compliance: Clients are expected to follow their prescribed treatment plan and adhere to any home exercise programs provided by their therapist.
Conduct: We expect all clients to conduct themselves respectfully towards staff and other clients. Any disruptive or inappropriate behavior may result in termination of services.
6. Privacy and Confidentiality
UDEA Physical Therapy Inc is committed to protecting your privacy. All client information is confidential and will only be shared with your consent or as required by law. Our full privacy policy is available upon request.
7. Termination of Services
We reserve the right to terminate services at any time, particularly if the terms outlined in this agreement are not followed.
8. Changes to Terms
UDEA Physical Therapy Inc may update these Terms of Service from time to time. We will notify clients of any significant changes.
